|TMSGA Team Registration Form - Each school must submit a completed registration form to the TMSGA office.
|TMSGA Co-Op Registration Form - Schools that do not have enough players to fill a team can complete this form and submit to TMSGA office for approval. A full boys' team is four (4) or more players; a full girls' team is two (2) or more players. Schools which cannot meet these thresholds are eligible for Co-Op consideration. Each school must be a member of the TMSAA or non-members. Current TMSAA approved Co-Ops do not need to be approved by TMSGA.
|TMSGA Parent Consent Form - Each parent must fill out and sign this form for their child to be able to participate. Each coach must gather each team member's form and fax/email to TMSGA office prior to the team's first regular season match.
| TMSGA Individual Registration Form - Any player wishing to participate as an individual will need to submit this form, or any school that does not have a full team and wants their students/athletes to participate as individuals will need to submit this form. Full teams do NOT need to submit this form for each player.
